Yes. You must bring a valid driver’s license or government-issued photo ID to your DOT physical. The medical examiner must verify your identity before completing the exam or issuing a medical certificate.
If you use a CPAP, bring a compliance report. If you’ve been referred for a sleep study, we’ll need results or documentation that meets DOT standards.
You must test using the corrective lenses you drive with. Bring your glasses or contacts (and a backup if you have one).
