Must bring (everyone)
Valid driver’s license (CDL/License)
Any glasses/contacts you drive with (bring them even if you “rarely use them”)
Hearing aids (if you use them)
List of all medications (name, dose, and prescribing provider)
Medical history details (surgeries, hospitalizations, diagnoses—be ready to explain)
Bring if you have a PCP/specialist
Recent visit notes or a brief letter can help (especially if you have ongoing conditions).
Not always required — but it can prevent delays.
Bring if you have a new diagnosis
If something changed recently (new meds, new symptoms, new diagnosis), bring documentation so the examiner can review safely.
Bring these if they apply to you
CPAP / sleep apnea: your compliance report (commonly last 30–90 days, if available)
Diabetes: most recent A1C (lab result or provider documentation)
Insulin-treated diabetes: any treating clinician paperwork you’ve been given for DOT. (Form
MCSA-5870)
High blood pressure: list of BP meds and any recent readings (if you track them)
Heart history: cardiology notes, stress test/echo summary if recently performed (when available)
Seizure history: neurology notes and medication list (if applicable)
Recent surgery or injury: return-to-work note and any restrictions
Bottom line: The cleaner your paperwork is, the faster your visit goes.
